Job Description

Clinical Pharmacist (MTM) Position Description: The Clinical Pharmacist is responsible for providing medication management and other clinical pharmacy services to optimize medication-related healthcare outcomes. The pharmacist primarily supports Medication Therapy Management (MTM) services and may provide additional clinical pharmacy services based on client and organizational needs.

Performance Objectives and Responsibilities:

Duties may include but are not limited to:

- Provide MTM services, including Comprehensive Medication Reviews (CMRs), Targeted Medication Reviews (TMRs), adherence interventions, cost-effective interventions, and other medication-related clinical interventions.

- Review medication therapy to identify and address medication-related problems, including safety concerns, drug interactions, therapeutic duplication, gaps in therapy, adherence barriers, and opportunities to optimize treatment.

- Provide patient education and counseling regarding medication therapy, adherence, and other medication-related concerns.

- Communicate clinical recommendations and interventions to prescribers and other members of the healthcare team.

- Conduct patient and provider outreach to support medication management services and resolution of identified medication-related concerns.

- Provide other clinical pharmacy services, including pharmacogenomic (PGx) consultations, adherence and quality initiatives, transitions of care, and other clinical programs as assigned.

- Accurately document clinical reviews, interventions, recommendations, outreach, and outcomes in applicable systems.

- Collaborate with pharmacy technicians and other members of the interdisciplinary team in the provision of medication management services.

- Maintain established clinical, quality, productivity, and documentation standards.

- Follow applicable clinical protocols, client requirements, policies, procedures, and federal and state regulations.

- Participate in training, quality improvement activities, and new clinical programs as needed.

- Perform other duties as assigned by the Director of Clinical Services, Manager, and Supervisor.

Qualifications and Requirements:

- Graduate of an accredited college of pharmacy with a Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) degree.

- Active pharmacist license in good standing, with the ability to obtain and maintain licensure in multiple states as required.

- Clinical pharmacy experience preferred, including experience in MTM, ambulatory care, population health, telepharmacy, medication adherence, pharmacogenomics, or other patient-centered pharmacy services.

- Strong clinical knowledge and ability to assess medication therapy and identify appropriate interventions.

- Excellent verbal and written communication skills, including the ability to effectively communicate with patients, prescribers, and other healthcare professionals.

- Strong organizational, time-management, critical-thinking, and problem-solving skills.

- Ability to work independently in a remote environment while collaborating effectively with a multidisciplinary team.

- Ability to learn and work across multiple clinical programs and adapt to evolving client and organizational needs.

- Proficient with computer systems, electronic health records, clinical documentation platforms, pharmacy software, and standard productivity applications. Work Environment & Physical Demands

- Remote/work-from-home position requiring a HIPAA-compliant workspace.

- Must be able to view a monitor, use a keyboard, communicate by telephone or video, and remain seated for extended periods.

- Frequent computer and telephone use with occasional repositioning of light office equipment.

If you will be working at home occasionally or permanently, the internet connection must be obtained through a cable broadband or fiber optic internet service provider with speeds of at least 10Mbps download/5Mbps upload.

For this position, we anticipate offering an hourly rate of 54 - 91 USD / hourly, depending on relevant factors, including experience and geographic location.

This role is also anticipated to be eligible to participate in an annual bonus plan.
